Six hidden reasons your enamel is losing minerals — and only one of them is about brushing

People who brush twice a day, floss, and do everything they were told still walk out of the surgery with a filling. The reasons are not mysterious. They are just never explained.
Enamel is a mineral. Minerals dissolve in acid. That one sentence explains more about why teeth fail than anything you were ever taught about technique.
There is a number for it: below a pH of about 5.5, enamel starts giving up mineral. Above it, mineral moves back in. Everything below is about what pushes you under that line, and how often.
You brush at the two moments furthest from the six that matter
Every time you eat or drink, plaque bacteria turn it into acid within seconds. pH falls for about five minutes, sits under the line for roughly fifteen more, and saliva then spends twenty minutes climbing back.
Breakfast, coffee, something mid-morning, lunch, an afternoon coffee, dinner. Six occasions is six acid attacks a day — and you brush first thing and last thing, the two moments furthest from all of them.
How often you eat matters more than how much
This is old, settled cariology and it still surprises people. A chocolate bar eaten in one sitting is one attack. The same bar eaten in six pieces across an afternoon is six.
Which means the careful person who grazes all day can be worse off than the one who has a large pudding and then leaves the mouth alone for four hours. Enamel does not count calories. It counts occasions.
Sparkling water and coffee are acid attacks too
Nothing here has to be sweet. Carbonated water is mildly acidic by definition — the bubbles are carbonic acid — and it sits close enough to the line to matter when it is sipped over an hour at a desk.
Coffee is acidic. So is tea with lemon. So is a glass of wine. Nobody is going to give these up, and this article is not going to suggest it. But they are attacks, and they are never counted as such.
The healthy swaps are often the most acidic things you drink
Lemon water first thing. Kombucha. Cider vinegar. Citrus. Sports drinks. Every one of these arrived in a diet as an upgrade, and every one of them is strongly acidic.
The upgrade is real for the rest of the body. It is not an upgrade for the enamel surface — and sipping it slowly, which is exactly what people do with a hot lemon water, is the worst possible way to take it.
A dry mouth removes both of your defences at once
Saliva does two jobs. It buffers the acid back up, and it carries the mineral that goes back into the surface. Lose saliva and you lose both at the same time.
Common causes: many prescription medications, breathing through the mouth at night, dehydration, stress — and, quietly, alcohol-based mouthwash. A product bought to make the mouth cleaner can leave it drier, and a drier mouth is worse at defending itself.
Everything in the cabinet subtracts. Almost nothing adds.
Look along the shelf and ask one question of each product: after you spit, is anything left on the tooth?
Brushes remove plaque. Rinses remove and kill. Whitening strips strip. Floss removes. All useful, all subtraction. None of them put mineral back into a surface that has just given some up — and mineral is precisely what the acid took.
And if all six keep running, this is what it costs you
This is the part that never makes it into the leaflet, and it is the only part that decides anything. None of it hurts until it is too late to undo. It arrives in the same order in almost everybody.
The surface roughens microscopically. Stain grabs on faster, so coffee shows sooner after a cleaning than it used to. The film along the lower front teeth is back by evening. Breath fades earlier in the day.
Cold starts registering — water, winter air. And the colour shifts, not because the teeth are dirtier but because they are thinner: enamel is translucent and the dentine underneath it is naturally, permanently yellow. Which is why whitening keeps disappointing people. It bleaches a surface whose problem was never stain.
Chalky white marks near the gumline. Those are demineralised patches and this is the last point at which mineral can still go back in. Almost nobody notices them and almost nobody has been told what they are.
The surface breaks, and now it is a drill. A filled tooth is never the original tooth again. Fillings get replaced, and each replacement takes a little more of what is left — until there is not enough left to fill and it becomes a crown, then a root canal.
Breath: sulphur compounds made by bacteria on the tongue, which is why a mint lasts two hours — and why an alcohol rinse makes it worse by drying the mouth that was washing them out.
Gums: blood in the sink is inflammation, not vigorous brushing. Gum disease, not decay, is the leading reason adults lose teeth, and it is painless until late.
Further: an untreated infection can travel out of the root into the jaw or the floor of the mouth. Uncommon, not historical. And gum disease is associated — that is the honest word — with cardiovascular disease and with worse blood-sugar control.
Daily: the closed-mouth smile in photographs, the hand up when you laugh, the half step back in a conversation. For most people that is the cost actually being paid, long before a dentist mentions anything.
So what actually goes back in?
Enamel is not made of “calcium” in a general sense. It is made overwhelmingly of one specific mineral: hydroxyapatite. When enamel loses mineral, that is the mineral it loses — so the idea is not a substitute or a coating, but the same material offered back, in a particle small enough to settle onto the surface.
The second half is the bacteria producing the acid. There the useful ingredient is xylitol: close enough to sugar that they take it up, and then unable to metabolise it. They fill up on something that gives them nothing.
Why a spray and not another rinse
It comes straight out of reason one. A toothbrush cannot go to lunch with you, and neither can a 500 ml bottle that has to be spat into a sink. The six attacks happen at a desk, in a car and in a meeting room.
So the format had to work with no water, no rinsing and no sink — which also means it has to be safe to swallow. That is the whole position: it is the one thing here built for the fourteen hours you are nowhere near a sink. Everything else in the cabinet needs one.
